Job summary

Resource 1 Homecare Staffing is seeking a Hospice Registered Nurse (RN) Case Manager in Oakdale, MN to plan, organize and direct hospice care with emphasis on community health and home-based services.

This role supports a caseload, collaborates with physicians and the interdisciplinary team, participates in on-call duties, and ensures quality care, patient comfort and family education.

Qualifications

• Current RN license in good standing.
• 1-2 years of acute care experience in an institutional setting required.
• 1-2 years of home care experience preferred.
• Hospice philosophy understanding and comfort with end-of-life care.

Responsibilities

• Perform initial and ongoing comprehensive patient assessments to determine hospice needs.
• Provide professional nursing care following the nursing process.
• Develop and revise the plan of care in collaboration with the inter-disciplinary team.
• Administer medications and treatments as prescribed.
• Counsel patient and family on care needs and self-care techniques.
• Coordinate patient care across home and facility settings; participate in on-call duties.
