Resource 1 Homecare Staffing in Eagan, MN is seeking a Hospice Registered Nurse (RN) Case Manager to provide compassionate, in-home hospice care for a caseload of patients.

Qualifications include RN license, 1–2 years acute care and home care experience, and a patient-centered approach.
Qualifications
- 1-2 years of recent acute care experience in an institutional setting (required). 1-2 years of recent home care experience preferred.
- Graduation from an accredited school of nursing; valid RN license.
- Strong hospice philosophy understanding and compassionate demeanor.
- Ability to work with minimal supervision and travel as needed.
Responsibilities
- Completes initial and ongoing patient and family assessments to determine hospice needs.
- Develops and revises the care plan and coordinates patient care.
- Administers medications and treatments as prescribed.
- Provides caregiver education and health counseling.
- Participates in on-call duties as defined by policy.
- Maintains accurate clinical notes and coordinates with physicians.
