Job summary

Allay Hospice is seeking a passionate Hospice RN to join our team in Wisconsin. As an RN Case Manager, you will deliver expert, patient-centered hospice care with strong interdisciplinary support and leadership that understands hospice nursing.

You will provide 1:1 care, collaborate with Social Work, Chaplaincy, Aides, and providers, and participate in after-hours triage, ensuring documentation is streamlined through HCHB EMR. Travel throughout the territory is required.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.

Responsibilities

• Assess/ monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

Education
