Job summary
Generations Hospice is seeking a dedicated Hospice RN Case Manager to deliver patient-centered care in the Fond du Lac area. You will assess needs, develop care plans, administer treatments, and support families within a collaborative team.
The role offers a strong work-life balance with regular hours and opportunities for advancement. Candidates should have an RN license and hospice experience; a $7,500 sign-on bonus is available for external hires.
Qualifications
- Must be a graduate of an accredited nursing program with RN licensure in the operating state.
- One year nursing experience required; three+ years preferred.
- Experience in hospice or similar setting is a plus.
Responsibilities
- Assess and monitor physical, emotional, and psychological needs of patients.
- Create hospice care plans aligned with patient wishes and goals.
- Direct nursing care: medications, treatments, and interventions.
- Provide pain and symptom management and patient education to families.
- Collaborate with an interdisciplinary team; maintain accurate documentation.
