Generations Hospice in Fond du Lac, WI seeks an experienced RN Case Manager to provide patient-centered hospice care. You will assess needs, develop care plans, and administer nursing care with strong team support and after-hours triage resources.

Qualifications

• Graduate of an accredited nursing program with current RN licensure in the operating state.
• Minimum 1 year nursing experience; 3+ years preferred.
• Hospice experience or similar setting preferred.
• Strong commitment to clinical documentation and care planning.

Responsabilités

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligning with patient wishes and goals.
• Direct nursing care: administer medications, treatments, interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
