Job summary

Trustbridge Hospice, a proud member of Empath Health, is seeking a compassionate Hospice Registered Nurse (RN) Case Manager to join our Palm Beach County team. Monday - Friday 8:30am - 5:00pm with a weekend requirement, providing patient-centered care and guiding families through the illness journey.

The role involves coordinating care with the primary physician and interdisciplinary team, developing individualized plans, and delivering education to improve comfort and quality of life.

Qualifications

• Licensed Florida RN in good standing.
• Current Basic Life Support certification.
• 1 year hospice, home health, oncology, medical/surgical, or geriatric nursing preferred.
• Understanding of hospice case management and crisis intervention.

Responsibilities

• Advocate for patients and families, addressing needs.
• Coordinate comprehensive care with the interdisciplinary team.
• Develop and implement individualized care plans (IDG).
• Monitor patient conditions and provide interventions.
• Deliver education to enhance comfort and quality of life.

Trustbridge Hospice, a proud member of Empath Health, is seeking a caring and motivated Hospice Registered Nurse (RN) Case Manager to join our dedicated hospice team in the Palm Beach County area. In this vital role, you will provide exceptional, patient-centered care while guiding families through one of life's most meaningful journeys on the following schedule: Monday - Friday 8:30am - 5:00pm with a weekend requirement.

What You'll Do

• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
• Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group)
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
• Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust

What You'll Need

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license.
