Job summary

Trustbridge Hospice, a member of Empath Health, is seeking an RN Hospice Case Manager for Broward County on a PRN basis. Shift 8:30am–5pm; 64 hours per month. You will coordinate patient/family care under the guidance of the physician and RN, focusing on comfort, dignity and individualized care plans.

Required Florida RN license; BSN preferred and hospice certification preferred. Strong communication and EMR skills are essential to support the interdisciplinary team and ensure high-quality,

Qualifications

• Registered Nurse licensed in Florida required.
• BSN preferred; hospice experience valued.
• Hospice certification preferred and BLS current.
• Strong communication, teaching and case-management skills.
• Familiarity with EMR systems and care coordination.

Responsibilities

• Coordinate patient/family care under the direction of the physician and RN.
• Identify patient/family needs and develop an individualized plan of care.
• Participate in the interdisciplinary group (IDG) and support care delivery.
• Utilize monitoring and teaching to maximize patient comfort and quality of life.
• Maintain confidentiality and provide compassionate, personalized service.

Trustbridge Hospice, a member of Empath, is currently seeking a caring and motivated RN Hospice Case Managerto join our team in Broward County on a PRN basis. What you'll Do

• Supports and carries out the organization’s mission, vision, and values on a consistent basis.
• Coordinate the care for the patient/family under the direction of the primary physician and registered nurse.
• Responsible for identifying patient/family needs.
• Coordinating the care for the patient/family.
• As part of the interdisciplinary group (IDG) contributes and follows an individualized plan of care
• Utilizes monitoring, intervention and teaching skills to maximize patient comfort and enhance the quality of life for the patient/family unit.
• Pledge to make the comfort and care of patients and families the highest priority.
• Consistently provide a high level of personalized, sincere and engaging service to patients and families at all times.
• Ensure the complete satisfaction of patients and families; promptly and effectively address all concerns.
• Maintains utmost respect for confidentiality in care of the patient/family.

What You'll Need

• Registered Nurse currently licensed in the State of Florida.
• BSN preferred
• Hospice certification preferred.
• Current Basic Life Support Certification.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
