Empath Health in Palm Beach County is seeking an RN Hospice Case Manager on a PRN schedule 8:30am-5pm with weekend availability to coordinate patient care under physician and IDG.

You will use your Florida RN license, BSN preferred, hospice experience, and EMR skills to develop an individualized plan of care and support families with dignity and comfort.

Qualifications

• Registered Nurse licensed in Florida.
• BSN preferred.
• Hospice certification preferred.
• Current Basic Life Support certification.
• Experience with hospice nursing: case management, crisis intervention, communication, teaching.

Responsibilities

• Coordinate care for the patient/family under the direction of the physician and RN.
• Identify patient/family needs and develop an individualized care plan.
• Work within the interdisciplinary group to maximize patient comfort and quality of life.
• Maintain confidentiality and provide compassionate, personalized service.

Empath Trustbridge Hospice is currently seeking a caring and motivated RN Hospice Case Manager to join our team in Palm Beach County. You would work the following schedule: PRN 8:30am - 5pm with weekend availability

What you'll Do

• Supports and carries out the organization’s mission, vision, and values on a consistent basis.
• Coordinate the care for the patient/family under the direction of the primary physician and registered nurse.
• Responsible for identifying patient/family needs.
• Coordinating the care for the patient/family.
• As part of the interdisciplinary group (IDG) contributes and follows an individualized plan of care
• Utilizes monitoring, intervention and teaching skills to maximize patient comfort and enhance the quality of life for the patient/family unit.
• Pledge to make the comfort and care of patients and families the highest priority.
• Consistently provide a high level of personalized, sincere and engaging service to patients and families at all times.
• Ensure the complete satisfaction of patients and families; promptly and effectively address all concerns.
• Maintains utmost respect for confidentiality in care of the patient/family.

What You'll Need

• Registered Nurse currently licensed in the State of Florida.
• BSN preferred
• Hospice certification preferred.
• Current Basic Life Support Certification.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
