Job summary

Empath Health's Trustbridge Hospice is seeking an RN Hospice Case Manager to join our Broward/Palm Beach County team. The role is PRN, 8:30am–5pm, with weekend availability and a focus on compassionate, patient-centered care.

You will collaborate with the IDG to assess needs, develop care plans, and support families through advanced illness with dignity and comfort. We offer comprehensive benefits, CEU support, and growth opportunities within our mission-driven organization.

Qualifications

• Current Florida RN license.
• BSN preferred.
• Hospice certification preferred.
• Active Basic Life Support (BLS).
• One year in public health, home health, oncology, medical/surgical, geriatric or hospice nursing preferred.
• Understanding of hospice case management, crisis intervention, and teaching skills.
• Basic computer skills and EMR experience a plus.
• Valid Florida driver’s license and reliable transportation.

Responsibilities

• Coordinate patient/family care under the direction of the primary physician and registered nurse.
• Identify patient/family needs.
• Coordinate care for the patient/family.
• Contribute to an individualized plan of care as part of the interdisciplinary group (IDG).
• Utilize monitoring and teaching to maximize comfort and quality of life for the patient/family.
• Maintain confidentiality and provide compassionate, personalized service to patients and families.

Empath Trustbridge Hospice is currently seeking a caring and motivated RN Hospice Case Manager to join our team in Broward County and southern Palm Beach County. You would work the following schedule: PRN 8:30am - 5pm with weekend availability.

What You'll Do

• Supports and carries out the organization’s mission, vision, and values on a consistent basis.
• Coordinate the care for the patient/family under the direction of the primary physician and registered nurse.
• Responsible for identifying patient/family needs.
• Coordinating the care for the patient/family.
• As part of the interdisciplinary group (IDG) contributes and follows an individualized plan of care
• Utilizes monitoring, intervention and teaching skills to maximize patient comfort and enhance the quality of life for the patient/family unit.
• Pledge to make the comfort and care of patients and families the highest priority.
• Consistently provide a high level of personalized, sincere and engaging service to patients and families at all times.
• Ensure the complete satisfaction of patients and families; promptly and effectively address all concerns.
• Maintains utmost respect for confidentiality in care of the patient/family.

What You'll Need

• Registered Nurse currently licensed in the State of Florida.
• BSN preferred
• Hospice certification preferred.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
