Job summary

Trustbridge Hospice seeks an RN Hospice Case Manager to join our Palm Beach County team. The role is PRN, 8:30am–5pm with weekend availability, focused on patient-centered care and compassion for patients and families.

You will coordinate care under the direction of the primary physician and RN, identify patient needs, and contribute to an individualized plan of care within the IDG. We offer competitive pay and extensive benefits.

Qualifications

• Florida RN license required.
• BSN preferred.
• Hospice certification preferred.
• Basic Life Support (BLS) certification required.
• Understanding of hospice nursing: case management, crisis intervention, communication, teaching, thanatology.

Responsibilities

• Coordinate care for the patient/family under the direction of the primary physician and registered nurse.
• Identify patient/family needs and develop an individualized plan of care.
• As part of the interdisciplinary group (IDG) contribute to and follow the plan of care.
• Utilize monitoring, intervention and teaching to maximize patient comfort and quality of life.
• Maintain confidentiality and ensure complete satisfaction of patients and families.

Empath Trustbridge Hospice is currently seeking a caring and motivated RN Hospice Case Manager to join our team in Palm Beach County. You would work the following schedule: PRN 8:30am - 5pm with weekend availability

What you’ll Do

• Supports and carries out the organization’s mission, vision, and values on a consistent basis.
• Coordinate the care for the patient/family under the direction of the primary physician and registered nurse.
• Responsible for identifying patient/family needs.
• Coordinating the care for the patient/family.
• As part of the interdisciplinary group (IDG) contributes and follows an individualized plan of care
• Utilizes monitoring, intervention and teaching skills to maximize patient comfort and enhance the quality of life for the patient/family unit.
• Pledge to make the comfort and care of patients and families the highest priority.
• Consistently provide a high level of personalized, sincere and engaging service to patients and families at all times.
• Ensure the complete satisfaction of patients and families; promptly and effectively address all concerns.
• Maintains utmost respect for confidentiality in care of the patient/family.

What You’ll Need

• Registered Nurse currently licensed in the State of Florida.
• BSN preferred
• Hospice certification preferred.
• Current Basic Life Support Certification.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
