Job summary

Trustbridge Hospice in Florida seeks a compassionate Field-Based Emergent Care RN to provide rapid-response visits across Palm Beach County. You will work early hours 6:00am-4:30pm with every other weekend, delivering patient-centered care in home and community settings.

As a Florida-licensed RN with BLS, you will triage urgent health conditions, collaborate with physicians and the interdisciplinary team, and educate patients and families to enhance comfort and quality of life.

Qualifications

• Registered Nurse licensed in Florida is required.
• Current Basic Life Support (BLS) certification is required.
• Understanding Hospice Nursing concepts including case management, crisis intervention, communication, teaching, and thanatology.

Responsibilities

• Assess, triage, and manage urgent and emergent health conditions in home/community settings.
• Work with physicians and teams to deliver timely, high-quality care and reduce ER visits.
• Advocate for patients and families, addressing their needs.
• Coordinate care under the primary physician and interdisciplinary team guidance.
• Monitor patient conditions, deliver interventions, and provide education to improve comfort and quality of life.

Trustbridge Hospice, part of Empath Health, is seeking a compassionate and motivated Registered Nurse (RN) to provide emergent hospice field visits throughout Palm Beach County. You will work the following schedule: 6:00am - 4:30pm and every other weekend

What you'll Do

The Field-Based Emergent Care Registered Nurse (RN) provides rapid-response, patient-centered clinical care to individuals in home and community settings. You will:

• Be responsible for assessing, triaging, and managing urgent and emergent health conditions outside of traditional clinical environments.
• You will operate with a high level of autonomy while collaborating with physicians, advanced practice providers, and interdisciplinary teams to deliver timely, high-quality care and prevent unnecessary emergency department visits or hospitalizations.
• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life

What You'll Need

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
