Job summary

Empath Health’s Trustbridge Hospice in Palm Beach County is seeking a compassionate RN Hospice Case Manager to coordinate patient and family care under the direction of the primary physician and the interdisciplinary group. The role includes planning individualized care, monitoring and teaching to maximize comfort, and ensuring confidentiality and high-quality service.

The ideal candidate is a Florida-licensed RN, with BSN preferred, hospice certification preferred, and BLS.

Qualifications

• Registered Nurse currently licensed in Florida.
• BSN preferred.
• Hospice certification preferred.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs.
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with valid Florida driver's license.

• Supports and carries out the organization’s mission, vision, and values on a consistent basis.
• Coordinate the care for the patient/family under the direction of the primary physician and registered nurse.
• Responsible for identifying patient/family needs.
• Coordinating the care for the patient/family.
• As part of the interdisciplinary group (IDG) contributes and follows an individualized plan of care
• Utilizes monitoring, intervention and teaching skills to maximize patient comfort and enhance the quality of life for the patient/family unit.
• Pledge to make the comfort and care of patients and families the highest priority.
• Consistently provide a high level of personalized, sincere and engaging service to patients and families at all times.
• Ensure the complete satisfaction of patients and families; promptly and effectively address all concerns.
• Maintains utmost respect for confidentiality in care of the patient/family.

You would work the following schedule: PRN 8:30am - 5pm with weekend availability

What You'll Do
