Job summary

Trustbridge Hospice, part of Empath Health, is seeking an RN Hospice Case Manager in the Boca area. The role focuses on coordinating patient care, advocating for families, and leading the interdisciplinary team to ensure comfort and dignity for patients facing advanced illness.

The position requires a Florida RN license and BLS certification, with preferred experience in hospice, home health, oncology, or geriatric nursing. Monday–Friday schedule with one weekend every 6 weeks.

Qualifications

• Registered Nurse licensed in Florida (RN) with active license.
• Current Basic Life Support (BLS) certification.
• Experience in hospice, home health, oncology, or geriatric nursing is preferred.

Responsibilities

• Coordinate care under physician direction and with the interdisciplinary group.
• Develop and implement individualized care plans.
• Monitor patient conditions and provide interventions and education to improve comfort and quality of life.
• Advocate for patients and families to address needs promptly and effectively.
• Engage with the team to ensure seamless care across the hospice journey.

Trustbridge Hospice, a member of Empath Health, is currently seeking a caring and motivated RN Hospice Case Manager to join our team in the Boca area. You would work the following schedule: Monday - Friday 8:30am - 5:00pm with a weekend requirement (One weekend every 6 weeks)

What You’ll Do

As an RN Case Manager, you will play a vital role in coordinating care and supporting patients and families throughout their hospice journey. You will:

• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
• Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group)
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
• Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust

What You’ll Need

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
