Job summary

JourneyCare Hospice is seeking a Hospice RN to join our team in Waukegan, IL. The position offers a Monday–Friday schedule with occasional weekend coverage and a competitive salary between $80,000 and $97,000 annually, based on experience.

The role includes direct patient care, coordination with a broad care team, and responsibilities from admission through ongoing assessment. We provide generous PTO, a 401(k) match, and tuition reimbursement to support professional growth.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Current CPR and First Aid certifications.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation of the nursing process.
• Obtain data and assess information from patient’s physical, psychological, social, and spiritual factors affecting needs; coordinate with the interdisciplinary team.
• Initiate communication with the attending physician, interdisciplinary team, and other agencies to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, reports and care plans timely.

What You’ll Do

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
