JourneyCare Hospice is seeking a Hospice RN to join our team in Morton Grove, IL. This role covers patient and family care across the nursing process, with competitive salaries and a comprehensive benefits package.

You will collaborate with an interdisciplinary team, ensure high-quality care, and participate in ongoing training and development opportunities. The position emphasizes compassionate end-of-life care and community-based visits.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR and First Aid certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver's license and proof of insurance required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation and evaluation of the nursing process.
• Obtain data from patient, including physical, psychological, social and spiritual factors, and coordinate care with the interdisciplinary team.
• Initiate communication with the attending physician and other agencies to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the plan of care.
• Perform clinical pronouncement of death and provide family support.
• Maintain accurate patient records, charts, and care plans in a timely manner.
• Supervise and document delivery of care by Home Health Aide as required by guidelines.

What You'll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
