JourneyCare Hospice seeks a compassionate Hospice RN to join our team in Morton Grove, IL. You will manage patient and family caseload, assess needs across physical, psychological, social and spiritual dimensions, and coordinate with the interdisciplinary team to ensure optimal care.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• CPR and First Aid certifications are required.
• Knowledge of quality assessment and assurance procedures.
• Valid driver's license and proof of insurance are required.

Responsibilities

• Take primary responsibility for patient and family caseload including assessment, eligibility, admission processing, planning, implementation and evaluation.
• Gather data from patient and family considering physical, psychological, social and spiritual factors and coordinate with the interdisciplinary team.
• Communicate with attending physician and other agencies to coordinate care.
• Provide regular visits to reassess patient needs and update the care plan.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records, charts and progress notes.
• Supervise and document delivery of care by Home Health Aide per guidelines.

Monday-Friday 8:30am-5pm and One Weekend Every 12th Week (4 weekends/year). 1 Winter Holiday and 1 Summer Holiday

What You'll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
