Job summary

Athena Health Care Systems’ Hospice Services of Massachusetts in the Plymouth area seeks a dedicated Hospice RN Case Manager to supervise aides, assess patient and family needs, and provide professional nursing care using all elements of the nursing process.

You will develop individualized care plans, collaborate with an interdisciplinary team, and educate patients and families on nursing care requirements and disease management.

Qualifications

• Graduate of an accredited school of nursing.
• Active and unencumbered RN license in Massachusetts.
• Prior hospice experience preferred.
• CPR certification.

Job description

We are seeking a dedicated and compassionate Hospice RN Case Manager to join our team in the Plymouth area. As a Hospice RN Case Manager with Hospice Services of Massachusetts, you will be responsible for supervising hospice aides, completing comprehensive assessments of patient and family needs, and providing professional nursing care utilizing all elements of the nursing process. You will develop and implement individualized care plans, collaborate with interdisciplinary team members, and provide health care instructions to patients and families.

Weekly pay up to $50 an hour, depending on experience.
