Job summary

Athena Health Care Systems is seeking an experienced Hospice RN Case Manager for the Massachusetts region. This on-the-road role requires travel within Barnstable and Plymouth Counties, with a driver’s license and reliable vehicle. Per diem schedule offers up to $50/hour with weekly pay.

Responsibilities include assessments, care planning, and interdisciplinary collaboration to support patients and families. Strong nursing background and MA RN license are essential.

Qualifications

• Graduate of an accredited school of nursing.
• Active and unencumbered RN license in Massachusetts.
• 1–2 years of acute care experience in an institutional setting; 1–2 years in home care preferred.
• CPR certification.

Per diem Hospice RN Case Managers with Hospice Services of Massachusetts

We are seeking an experienced and compassionate RN Case Manager to join our hospice team. As a Hospice RN Case Manager, you will plan, organize, and direct hospice care for our patients and their families.

Up to $50 an hour, depending on experience, with weekly pay.

This is an on-the-road position that requires the candidate to have a driver's license and an insured and reliable vehicle. Candidate must be willing to travel to portions of Barnstable and Plymouth Counties.

• Provide professional nursing care by utilizing all elements of the nursing process


• Develop and implement individualized care plans

Requirements

• Must possess an active and unencumbered RN license in Massachusetts


• 1-2 years of recent acute care experience in an institutional setting, and 1-2 years of recent experience in home care preferred
