Job summary

Hospice Services of Massachusetts is seeking a dedicated per diem Hospice RN Case Manager in the North Easton area. You will supervise hospice aides, perform comprehensive assessments, and provide nursing care using all elements of the nursing process.

You will develop individualized care plans and collaborate with an interdisciplinary team while educating patients and families about care needs. Prior hospice experience preferred; MA RN license and CPR required.

Qualifications

• Graduate of an accredited school of nursing.
• Must possess an active Massachusetts RN license.
• Prior hospice experience preferred.
• CPR certification required.
