Job summary

Hospice of Western & Central Massachusetts is seeking a compassionate Hospice RN Case Manager to join our care team. You will deliver comprehensive hospice services to patients and families in homes and communities, building strong relationships and coordinating care.

The role operates Monday through Friday, 8:00 am–4:30 pm, with on-the-road responsibilities and mileage reimbursement. Compensated at up to $49 per hour with weekly pay and opportunities for advancement within a supportive team.

Qualifications

• 1-2 years of acute care experience in an institutional setting and 1-2 years in home care.
• CPR certification required.
• Active and unencumbered RN license in Massachusetts.
• BSN with hospice care experience preferred.

Responsibilities

• Plan, organize, and direct hospice care for patients and families in their homes and communities.
• Conduct initial, comprehensive, and ongoing assessments to determine patient and family needs.
• Develop and implement patient care plans, collaborating with patients, families, and healthcare providers.
• Provide professional nursing care, utilizing elements of the nursing process.
• Supervise hospice aides and revise care plans as necessary.
• Collaborate with healthcare providers to ensure patient continuity of care.
• Educate patients and families on disease management, wound care, and symptom management.
• Participate in patient care conferences to review progress and adjust care plans.

As a key member of our care team, you will provide exceptional hospice care to patients and their families, building relationships and delivering comprehensive services that support their unique needs.
