Athena Health Care Systems seeks an experienced per diem RN Case Manager to join our hospice team. You will plan, organize, and direct hospice care for patients and families while traveling in Plymouth County and portions of the Cape.
Candidates must have an active MA RN license, be CPR certified, graduate from accredited nursing program, and be willing to travel with a reliable vehicle. Hospice experience preferred.
We are seeking an experienced and compassionate per diem RN Case Manager to join our hospice team. As a Hospice RN Case Manager, you will plan, organize, and direct hospice care for our patients and their families.
Up to $50 an hour, depending on experience, with weekly pay.
This is an on-the-road position that requires the candidate to have a driver's license and an insured and reliable vehicle. Candidate must be willing to travel throughout Plymouth County and portions of the Cape.
Supervise hospice aides
Complete comprehensive assessments of patient and family to determine hospice needs
Provide professional nursing care by utilizing all elements of the nursing process
Develop and implement individualized care plans
Collaborate with interdisciplinary team members to ensure comprehensive care
Provide health care instructions to patients and families
Counsel patients and families on meeting nursing and related needs
Graduate of an accredited school of nursing
Must possess an active and unencumbered RN license in Massachusetts
Prior hospice experience preferred
CPR certified
