Job summary

Hospice of Western & Central Massachusetts is seeking a compassionate Hospice RN Case Manager to join our team. You will provide full hospice care to patients and families in homes and communities, coordinating with the care team.

This on-road position offers mileage reimbursement, shift is Monday–Friday 8a–4:30p, and up to $49/hour with weekly pay. Opportunities for career advancement and comprehensive benefits provided.

Qualifications

• 1–2 years of acute care experience in an institutional setting.
• 1–2 years of home care experience.
• CPR certification required.
• Active RN license in Massachusetts required.
• Bachelor's degree in Nursing preferred with hospice care experience.

Responsibilities

• Plan, organize, and direct hospice care for patients and families in homes and communities.
• Conduct initial and ongoing assessments to determine patient/family needs.
• Develop and implement patient care plans with patients, families, and providers.
• Provide professional nursing care using the nursing process.
• Supervise hospice aides and revise care plans as needed.
• Collaborate with healthcare providers to ensure continuity of care.
• Educate patients and families on disease management, wound care, and symptom management.
• Participate in patient care conferences to review progress and adjust plans.

Job description

As a key member of our care team, you will provide exceptional hospice care to patients and their families, building relationships and delivering comprehensive services that support their unique needs.

Shift is Monday through Friday 8a-4:30p. This is an on-the-road position with mileage reimbursement. Up to $49 an hour with weekly pay.

• Supervise hospice aides and participate in revising patient care plans as necessary

• Provide education and guidance to patients and families on disease management, wound care, and symptom management
• Participate in patient care conferences to review patient progress and make adjustments to care plans

Requirements

• 1-2 years of recent acute care experience in an institutional setting and 1-2 years of recent experience in home care
• CPR certification
• Active and unencumbered RN license in Massachusetts
• Bachelor's degree in Nursing with 1 year of hospice care experience preferred
