Job summary

Interim HealthCare in Michigan is seeking a compassionate Hospice RN Case Manager to provide skilled home-based care. You’ll work with patients and families, coordinating with a physician team while enjoying autonomy and flexible scheduling.

The role requires an active Michigan RN license, CPR certification, and at least one year of clinical RN experience (prefer hospice). Travel is required to visit patients in their homes, with comprehensive benefits and opportunities for growth.

Responsibilities
