Job summary

Interim HealthCare is seeking a dedicated Hospice RN Case Manager in Michigan. You will provide skilled home-based nursing care, oversee a team of LPNs, CNAs, and aides, and coordinate patient care from admission to discharge.

Ideal candidates have an active RN license, hospice experience, CPR certification, and reliable transportation. This role offers autonomy, a strong work-life balance, and opportunities for bonuses.

Qualifications

• Active RN license in Michigan with unrestricted status.
• At least 1 year of clinical RN experience; hospice experience preferred.
• Current CPR certification and reliable transportation are required.

Responsibilities

• Provide home-based nursing care to patients in their homes.
• Oversee LPNs, CNAs, and aides delivering patient care.
• Assess patients, document progress, and report changes to physicians.
• Educate patients and families on care plans, medications, diet, and safety.
• Coordinate discharge planning and ensure goals are met.
