Job summary

Interim HealthCare is seeking a compassionate Hospice RN Case Manager to deliver skilled nursing care in patients' homes across Michigan. You will manage care plans, coordinate with physicians, and guide family members on home safety and discharge steps.

The role emphasizes autonomy, flexible assignments, and work-life balance while providing bonus opportunities and comprehensive benefits. A Michigan RN license and CPR are required.

Qualifications

• Active, unrestricted RN license in Michigan.
• Minimum 1 year clinical RN experience (Hospice preferred).
• Current CPR certification.
• Reliable transportation and valid driver's license.

Responsibilities

• Provide home-based care to patients in their homes.
• Oversee LPNs, CNAs and aides providing patient care as part of a team.
• Perform patient assessments, document progress, and report changes to the physician.
• Educate patients on plan of care, medications, therapy, diet and exercise.
• Coach family members on the care plan, safety, and discharge planning.
