Interim HealthCare Hospice in Michigan is seeking a Hospice Registered Nurse (RN) to deliver compassionate, home-based care to patients with six months or less to live. You will join an interdisciplinary team and perform assessments, monitor vitals, administer medications, and educate families.

Qualifications

• RN with active, unrestricted license in Michigan
• At least 1 year of RN experience (required)
• Community health and Medical-Surgical experience (preferred)

Responsibilities

• Provide home-based care to patients with six months or less to live
• Work as part of an interdisciplinary team with physicians, LPN, CNA, HHA, social worker, chaplain and volunteers
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medications and treatments
• Educate family on prognosis, changes in care plan, medications and hospice goals
• Take a holistic approach to patient and family care
