Job summary

Interim HealthCare Hospice in Grand Rapids, MI, is seeking a compassionate Hospice Registered Nurse (RN) for an on-call schedule: 7 days on (4:30PM-8AM Mon-Fri, 24/7 Sat-Sun) then off 7 days. You’ll provide home-based palliative care as part of an interdisciplinary team, perform assessments, monitor vitals, manage pain, educate families, and ensure supplies and equipment.

Active MI RN license required; 1 year home health experience preferred.

Qualifications

• Active, unrestricted RN license in Michigan.
• At least 1 year of home health experience preferred.
• 1 year of management experience preferred.

Responsibilities

• Provide home-based hospice care to patients with ≤6 months to live.
• Perform patient assessments and monitor vitals; assess breathing and pain.
• Educate family on prognosis, care plans, and hospice goals.
• Ensure accessibility of equipment, oxygen, medications and supplies.
• Collaborate with an interdisciplinary team to carry out the plan of care.
