JourneyCare Hospice in Morton Grove, IL is seeking a dedicated Hospice RN to manage patient and family care across the nursing process, including assessment, admission processing, planning, implementation, and evaluation phases.

You will collaborate with an interdisciplinary team to coordinate care, perform regular patient visits, update the care plan, document accurately, and support families during difficult times. The schedule includes Monday–Friday 8:30am–5pm with one weekend every 12th week.

Competenze

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• CPR and First Aid certifications required.
• Knowledge of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance required.

Mansioni

• Assume primary responsibility for patient and family caseload across the nursing process.
• Obtain data from patient and family to inform care needs and engage interdisciplinary team.
• Coordinate care by communicating with attending physician and other agencies.
• Provide regular visits to reassess patient needs and update the plan of care.
• Perform clinical pronouncement of death and support family.
• Maintain accurate patient records and care plans in a timely manner.
• Supervise delivery of care by Home Health Aide as required by guidelines.

HOURS:

Monday-Friday 8:30am-5pm and One Weekend Every 12th Week (4 weekends/year). 1 Winter Holiday and 1 Summer Holiday

What You’ll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
