Job summary

The Care Team Hospice is seeking a compassionate and patient-focused Hospice RN Case Manager to join our team. This role focuses on coordinating patient care, managing plans of care, and supporting families with comfort and quality of life.

Based in Indiana, you will work with physicians and an interdisciplinary team to deliver end-of-life care and education, while maintaining thorough documentation in Homecare Homebase.

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions with accurate documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care.

Schedule & Work-Life Balance

• Monday–Friday, 8:00 AM – 4:30 PM
• On-call opportunities available with additional compensation
• Some locations offer dedicated on-call teams, resulting in backup coverage only
