JourneyCare Hospice in Morton Grove, IL is seeking a compassionate Hospice RN to join our team. The role offers competitive salaries, generous time off, 401K with match, and extensive benefits.

You will provide end-of-life nursing care and coordinate with an interdisciplinary team. Responsibilities include assessing patients and families, developing and updating care plans, communicating with physicians, and mentoring home health staff.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Current CPR and First Aid certification.
• Knowledgeable of quality assessment and assurance procedures.
• Valid driver's license with proof of insurance.

Responsibilities

• Take primary responsibility for patient and family caseload, including assessment, admission processing, planning, implementation, and evaluation of the nursing process.
• Collect data from patient and family to identify needs across physical, psychological, social, and spiritual factors and coordinate care with the interdisciplinary team.
• Communicate with the attending physician and other agencies to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the care plan.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records and timely care plans.
• Supervise and document delivery of care by Home Health Aide per guidelines.

What You'll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
