Job summary

Addus HomeCare's JourneyCare Hospice unit in Illinois seeks a dedicated Hospice RN to join our team. You will manage patient and family caseload, coordinate with the interdisciplinary team, and provide compassionate end-of-life care in a home health setting.

Qualifications

• Graduation from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR and First Aid Certification.
• Knowledge of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance required.

Responsibilities

• Assume primary responsibility for patient and family caseload across assessment, eligibility, admission, planning, implementation and evaluation phases of nursing care.
• Collect data from patient and family to identify needs affecting care and coordinate with the interdisciplinary team.
• Coordinate care with attending physician and other agencies to ensure optimal outcomes.
• Provide regular visits to reassess patient needs and update the plan of care.
• Perform clinical pronouncement of death and provide family support.
• Maintain accurate patient records, charts, progress notes, and timely care plans.
• Supervise and document delivery of care by the Home Health Aide per guidelines.

HOURS

Salary:

$80,000-$97,000 Annually - Offer Based on Years of Experience

What You’ll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
