LilyCare Hospice of New Jersey is seeking a full-time Hospice Registered Nurse Case Manager to join our clinical team in Florham Park. You will manage a caseload of patients in private homes and facilities, delivering compassionate, individualized plans of care and routine visits.

You will coordinate pain and symptom management with the attending physician and Hospice Medical Director, participate in IDG meetings, educate families, and ensure accurate EMR documentation.

Competenze

• Current RN license in New Jersey or a compact state.
• Two years of RN experience; hospice, home health, or case management preferred.
• Current CPR certification.
• Valid driver’s license.

LilyCare Hospice is a community-based hospice serving patients and families across northern New Jersey, based out of Florham Park. We are hiring a full-time Hospice Registered Nurse Case Manager to join our clinical team and help shape hospice care delivery from the ground up.

Duties and Responsibilities

• Serve as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities - complete assessments, individualized plans of care, and routine visits
• Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director
• Participate in IDG meetings and coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy

Compensation and Schedule

The Hospice Registered Nurse Case Manager role pays $90,000 - $105,000/year based on experience. Full-time, Monday-Friday day schedule with a rotating on-call assignment. Territory is based out of Florham Park, NJ.
