Job summary

LilyCare Hospice of New Mexico is seeking a full-time Hospice Registered Nurse Case Manager to join our clinical team in central New Mexico. You will manage a caseload of hospice patients in private homes and facilities, delivering individualized plans of care and routine visits.

Responsibilities include pain and symptom management, education for patients and families, coordination with the interdisciplinary team, and timely EMR documentation.

Qualifications

• Active RN license in New Hampshire or a compact state.
• One year of RN experience; hospice/home health/case management preferred.
• Current CPR certification.

• Serve as the Hospice RN Case Manager for a caseload in private homes and facilities, performing assessments and plans of care.
• Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director.
• Coordinate care across the interdisciplinary team and supervise hospice aide care per the plan of care.
• Complete timely EMR documentation and participate in RN Case Manager on-call rotation.

LilyCare Hospice is a community-based hospice serving patients and families across central New Mexico. We are hiring a full-time Hospice Registered Nurse Case Manager to join our clinical team and help shape hospice care delivery from the ground up.

Duties and Responsibilities

• Serve as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities — complete assessments, individualized plans of care, and routine visits

• Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director

• Participate in IDG meetings and coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy

• Educate and support patients, families, and caregivers, and supervise hospice aide care per the plan of care

• Complete timely, accurate EMR documentation and participate in the RN Case Manager on‑call rotation

Compensation and Schedule

The Hospice Registered Nurse Case Manager role pays $90,000 - $105,000/year based on experience. Full-time, Monday–Friday day schedule with a rotating on‑call assignment. Territory is based out of Albuquerque, NM.
