Job summary

LilyCare Hospice in southern New Hampshire is seeking a full-time Hospice Registered Nurse Case Manager to join our clinical team. The role covers a southern New Hampshire territory based out of Bedford, NH, with a Monday–Friday day schedule and on-call rotation.

Responsibilities include assessing patients at home or facilities, coordinating care with the interdisciplinary team, and providing pain and symptom management under the guidance of the Medical Director.

Qualifications

• Current RN license in New Hampshire or a compact state.
• One year of RN experience required; hospice, home health, or case management preferred.
• Current CPR certification.
• Valid driver's license.

Responsibilities

• Serve as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities — complete assessments, individualized plans of care, and routine visits.
• Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director.
• Participate in IDG meetings and coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy.
• Educate and support patients, families, and caregivers, and supervise hospice aide care per the plan of care.
• Complete timely, accurate EMR documentation and participate in the RN Case Manager on-call rotation.

Duties and Responsibilities

• Serve as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities — complete assessments, individualized plans of care, and routine visits
• Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director
• Participate in IDG meetings and coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy

Compensation and Schedule

The Hospice Registered Nurse Case Manager role pays $90,000 - $105,000/year based on experience. Full-time, Monday–Friday day schedule with a rotating on-call assignment. Territory is southern New Hampshire, based out of Bedford, NH.
