Job summary

LilyCare Hospice in Albuquerque, NM seeks a dedicated Hospice Registered Nurse Case Manager to lead a caseload in private homes and facilities. Full-time, Monday–Friday day schedule with rotating on-call; territory based out of Albuquerque, NM. Competitive salary range and comprehensive benefits.

Responsibilities include assessments, care planning, pain and symptom management, and coordination with the interdisciplinary team; EMR documentation and on-call involvement are required.

Qualifications

• Current RN license in New Hampshire or a compact state.
• One year of RN experience required; hospice, home health, or case management experience preferred.
• Current CPR certification.
• Valid driver's license.

• Serve as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities — complete assessments, individualized plans of care, and routine visits
• Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director
• Participate in IDG meetings and coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy
• Educate and support patients, families, and caregivers, and supervise hospice aide care per the plan of care
• Complete timely, accurate EMR documentation and participate in the RN Case Manager on‑call rotation

LilyCare Hospice is a community-based hospice serving patients and families across central New Mexico. We are hiring a full-time Hospice Registered Nurse Case Manager to join our clinical team and help shape hospice care delivery from the ground up.

Duties and Responsibilities

• Serve as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities — complete assessments, individualized plans of care, and routine visits

• Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director
