Job summary

LilyCare Hospice is seeking a full-time Hospice Registered Nurse Case Manager to join our clinical team. You'll manage hospice care delivery for patients in their homes and facilities across southern New Hampshire.

This role involves providing skilled pain management, coordinating care with interdisciplinary teams, and completing EMR documentation. A strong RN background is essential, and you will have direct access to clinical leadership.

Qualifications

• Current RN license in New Hampshire or a compact state.
• One year of RN experience required; hospice, home health, or case management experience preferred.
• Current CPR certification.
• Valid driver's license.

Responsibilities

• Serve as the Hospice Registered Nurse Case Manager for hospice patients in private homes and facilities.
• Provide skilled pain and symptom management in coordination with the attending physician.
• Participate in IDG meetings and coordinate care across the interdisciplinary team.
• Educate and support patients, families, and caregivers.
• Complete timely, accurate EMR documentation and participate in the on-call rotation.

LilyCare Hospice is a community-based hospice serving patients and families across southern New Hampshire. We are hiring a full-time Hospice Registered Nurse Case Manager to join our clinical team and help shape hospice care delivery from the ground up.

Duties and Responsibilities

• Serve as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities — complete assessments, individualized plans of care, and routine visits




• Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director




• Participate in IDG meetings and coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy




• Educate and support patients, families, and caregivers, and supervise hospice aide care per the plan of care




• Complete timely, accurate EMR documentation and participate in the RN Case Manager on-call rotation

Compensation and Schedule

The Hospice Registered Nurse Case Manager role pays $90,000 - $105,000/year based on experience. Full-time, Monday–Friday day schedule with a rotating on-call assignment. Territory is southern New Hampshire, based out of Bedford, NH.
