Job summary

LilyCare Hospice in southern New Hampshire is hiring a full-time Hospice Registered Nurse Case Manager to lead care for patients in private homes and facilities. You will complete assessments, develop plans of care, and provide routine visits as part of a supportive clinical team.

Responsibilities include pain and symptom management with the attending physician and Hospice Medical Director, participating in IDG meetings, educating families, and handling EMR documentation.

Qualifications

• Current RN license in New Hampshire or a compact state.
• One year of RN experience required; hospice, home health, or case management experience preferred.
• Current CPR certification.
• Valid driver's license.

• Educate and support patients, families, and caregivers, and supervise hospice aide care per the plan of care.
• Complete timely, accurate EMR documentation and participate in the RN Case Manager on-call rotation.

Duties and Responsibilities

• Serve as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities — complete assessments, individualized plans of care, and routine visits
• Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director
• Participate in IDG meetings and coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy

Compensation and Schedule

The Hospice Registered Nurse Case Manager role pays $90,000 - $105,000/year based on experience. Full-time, Monday–Friday day schedule with a rotating on-call assignment. Territory is southern New Hampshire, based out of Bedford, NH.
