Job summary

LilyCare Hospice, serving northern New Jersey from Florham Park, seeks a Hospice Registered Nurse Case Manager to join our clinical team and help shape hospice care delivery across the region.

This full-time, on-site role offers a Monday–Friday daytime schedule with rotating on-call. Salary ranges from $90,000 to $105,000 per year, commensurate with experience, plus comprehensive benefits and mileage reimbursement.

Qualifications

• Current RN license in New Jersey or a compact state.
• Two years of RN experience required; hospice, home health, or case management experience preferred.
• Current CPR certification.
• Valid driver's license.

Responsibilities

• Serve as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities - complete assessments, individualized plans of care, and routine visits
• Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director
• Participate in IDG meetings and coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy
• Educate and support patients, families, and caregivers, and supervise hospice aide care per the plan of care
• Complete timely, accurate EMR documentation and participate in the RN Case Manager on-call rotation

LilyCare Hospice is a community-based hospice serving patients and families across northern New Jersey, based out of Florham Park. We are hiring a full-time Hospice Registered Nurse Case Manager to join our clinical team and help shape hospice care delivery from the ground up.

Duties and Responsibilities

• Serve as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities - complete assessments, individualized plans of care, and routine visits
