Job summary

LilyCare Hospice, based in Florham Park, NJ, is hiring a full-time Hospice Registered Nurse Case Manager to join our clinical team and help shape hospice care delivery from the ground up.

The role requires RN licensure in NJ or a compact state, 2 years of RN experience ( hospice/home health/case management preferred ), CPR certification, and a valid driver’s license. It includes a day schedule with rotating on-call and a Florham Park territory.

Qualifications

• Current RN license in New Jersey or a compact state.
• Two years of RN experience required; hospice, home health, or case management experience preferred
• Current CPR certification
• Valid driver's license

Responsibilities

• Serve as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities - complete assessments, individualized plans of care, and routine visits
• Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director
• Participate in IDG meetings and coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy
• Educate and support patients, families, and caregivers, and supervise hospice aide care per the plan of care
• Complete timely, accurate EMR documentation and participate in the RN Case Manager on-call rotation
