Job summary

Chapters Health System seeks a dedicated RN to provide direct patient care and coordinate the Plan of Care within the Interdisciplinary Group. The role emphasizes compassionate communication with patients and families, documentation, and collaboration with physicians and staff.

This position requires active RN licensure, BLS, and flexibility to meet the needs of Hospice House patients. Travel and on-call expectations may apply, with a focus on ensuring safe, dignified end-of-life care.

Qualifications

• Current license as RN in the state where the employee will be working
• Active BLS for healthcare professionals from the American Heart Association or Red Cross
• Minimum one (1) year nursing experience in hospice or hospital experience preferred
• Previous experience working with an EMR/EHR system

Responsibilities

• Provides and manages direct care to patients and families as part of Interdisciplinary Group (IDG).
• Coordinates the Plan of Care and ensures an individualized plan reflects evolving needs.
• Educates patient and family about disease, hospice care, and safety practices.
• Reports changes in condition to IDG and other health professionals.
• Attends patient deaths and oversees discharge processes.
• Maintains accurate documentation and communicates with physicians and staff using SBAR.
• Supervises nursing staff and contract nurses as needed.
• During emergencies may be required to report to work at designated locations.
