Job summary

Chapters Health System is seeking an RN for Hospice House to assess patient and family needs, coordinate care with the IDG, and provide palliative and supportive services. The role requires current RN licensure and hospice experience where possible, with strong communication skills.

Responsibilities include education, care planning, and collaboration with other health professionals, while maintaining compliance with policies and safety standards.

Qualifications

• Current license as RN in the state providing care or an active multi-state license.
• Minimum 1 year inpatient nursing or hospice experience; internal 6 months LPN in affiliate.
• Experience with EMR/EHR system preferred.
• Ability to communicate effectively in English (verbal and written).
• Flexible schedule to meet Hospice House patient needs.
• Ability to travel off-site as needed.

Responsibilities

• Provide and manage direct care to patients and families as part of the IDG.
• Coordinate the Plan of Care reflecting evolving patient needs.
• Educate patient and family about disease, palliation, and safety practices.
• Report changes in patient condition to IDG members.
• Participate in evaluating referrals/admissions for level of care.
