Job summary

Health Here is seeking an experienced Hospice RN for on-call duties to provide dignified, pain-managed end-of-life care. You will apply nursing theories to patient care, coordinate with families and the hospice team, and ensure documentation meets reimbursement guidelines.

Ideal candidates hold active RN licensure in the state, with at least two years of clinical nursing experience (hospice preferred) and a commitment to weekend, evening, and holiday call rotations.

Qualifications

• Current RN licensure in the state of practice.
• Minimum of two years clinical nursing experience; hospice experience preferred.
• Knowledge of home health and hospice care operations and the nurse’s role within the team.
• Understanding of nursing practices, agency policies, and reimbursement guidelines.
• Maintain 12 hours of annual continuing education and CPR certification.
• Willingness to travel and participate in weekend, evening, and holiday call rotations.

Responsibilities

• Apply nursing theories and concepts to patient care.
• Ensure documentation aligns with reimbursement guidelines.
• Coordinate physical care with patients, families, physicians, and the hospice team.
• Communicate effectively with patients, families, and agency personnel.
• Participate in interdisciplinary team meetings and quality reviews.
