Job summary

Chapters Health is seeking a Registered Nurse for Hospice House to assess patients, coordinate the Plan of Care with the IDG, and deliver compassionate palliative and supportive care to patients and families.

The ideal candidate has an active RN license, hospice or inpatient experience, and EMR experience. A flexible schedule, strong communication, and ability to handle emotional stress are essential for success in this role.

Qualifications

• Must hold an RN license valid in the state of care with active status.
• One year of inpatient nursing or hospice experience preferred; internal LPN experience considered for IPU.
• Experience with an EMR/EHR system preferred; strong English communication skills.
• BLS certification required for healthcare professionals; driver’s license and ability to travel may be needed.

Responsibilities

• Provide direct care to patients and families as part of the Interdisciplinary Group (IDG).
• Coordinate the Plan of Care reflecting evolving patient needs.
• Educate patients and families about disease, palliative care, and safety practices.
• Report changes in condition to IDG; participate in care level reviews for hospice referrals.
• Supervise nursing staff and other team members; perform other duties as assigned.
