Job summary

Chapters Health System seeks a registered nurse to admit patients to hospice services, coordinate the Plan of Care, and provide direct care alongside an interdisciplinary team. The role requires an active RN license, BLS, and experience with EMR/EHR systems.

A valid driver's license and transportation are also required to meet visit schedules. Flexibility in scheduling is needed to support Hospice House patients, including capability to handle emotional stress and physical demands associated

Qualifications

Current licensed RN in the state of employment.
Active BLS for healthcare professionals.
Minimum one year nursing experience in hospice or hospital preferred.
EMR/EHR experience required.
Must have a valid driver's license and auto insurance.

Responsibilities

Completes clinical admissions for all company services and educates patients and families empathetically.
Obtains physician orders and initiates Plan of Care for immediate needs.
Delivers skilled nursing interventions to prevent complications and maximize comfort.
Documents per policy and communicates with IDG and hospitals for referrals.
Represents the company professionally and adheres to regulatory requirements.
Performs patient care duties and collaborates with interdisciplinary team.
