Job summary

Christus Hospice and Palliative Care, part of the Optum family, is seeking a Patient Care Manager - Hospice to support Executive leadership in delivering safe, compliant patient care and to lead teams across home-based hospice services. This role requires an RN license in the state, at least 1 year of clinical experience, and a valid driver's license with reliable transportation.

CPR certification is preferred; 4+ years as an RN is a plus.

Qualifications

• Current unrestricted RN licensure in state of practice.
• 1+ years of clinical experience.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification preferred (or ability to complete within 90 days of hire).

Responsibilities

• Ensure compliance with hospice policies and with local, state, and federal regulations governing the assigned team.
• Direct and promote quality of care and safety for patients and staff.
• Engage in thorough problem resolution and complaint investigation.
• Provide in-services to staff on health and safety, regulatory issues, and agency policy and procedures.
• Ability to function in any home situation regardless of client background.
