Job summary

The position requires current RN licensure, hospice/home care background, and demonstrated leadership in an interdisciplinary environment, with proficiency in EMR workflows and adherence to professional standards.

Qualifications

• RN with current state licensure required.
• BSN preferred or minimum 1 year nursing experience in last 3 years.
• Hospice/home care experience with 2 years of management or supervisory experience.
• Complies with accepted professional standards and practice.
• HCHB and PointCare experience preferred.
• Excellent observation, nursing judgment, and communication skills; understands hospice care.
• Understands pain and symptom management principles.
• Licensed driver with insured vehicle in good standing.
• Maintains CPR certification.
• Demonstrates strong observation and written communication skills.

Responsibilities

• Receives case referrals, assigns hospice personnel, and consults with Medical Director on eligibility.
• Reviews and evaluates cases via home visits, conferences, and records; guides clinicians for quality home care.
• Reviews patient diagnoses, medications, procedures, and clinical course.
• Performs EMR records management and workflow in the system.
• Assists in establishing immediate and long-term goals and plan of care.
• Attends case conferences to coordinate care and interdisciplinarity.
• Conducts quarterly record reviews and communicates findings to leaders and staff.
• Assists in screening and interviewing new employees; helps orient new staff.
• Supports development and evaluation of training, policies, and organizational goals.
• Ensures compliance with professional standards and practices.
• Engages in community health assessment and program development.
• Participates in quality improvement teams and activities.
• Performs other duties as delegated by leadership.
