Job summary

Affinity Hospice Management LLC seeks an experienced Clinical Manager to coordinate patient care and supervise clinical staff in a hospice setting. You will review cases, manage EMR workflows, and guide clinicians to deliver high-quality home care services.

The role requires RN licensure in good standing, a BSN preferred, and experience in hospice or home care with supervision of staff in an interdisciplinary team. Strong communication and CPR certification are essential.

Qualifications

• Must be a licensed RN in the state.
• BSN degree preferred or 1 year of nursing management experience.
• Experience in hospice/home care and supervision in an interdisciplinary setting.

Responsibilities

• Receives and assigns case referrals with appropriate hospice personnel.
• Guides clinicians to improve home care services and quality.
• Reviews medical records and manages EMR workflow.
• Assists in setting goals and plans of care.
• Attends case conferences for care coordination.
• Conducts quarterly record reviews and communicates findings.
• Assists in interviewing and orientation of new staff.
• Supports inservice and education program planning and evaluation.
• Contributes to policy development and long-range planning.
• Promotes adherence to professional standards and CPR compliance.
• Engages in community health needs assessment and public relations.
• Participates in quality improvement teams and activities.
• Performs other duties as assigned.
