Qualifications

• Must hold a current RN license in the state.
• BSN preferred; at least 1 year of nursing experience in the last 3 years.
• Previous hospice/home care management experience is required.
• EMR experience with HCHB and PointCare is preferred.
• Must have a valid driver’s license and reliable vehicle.
• CPR certification current.

Responsibilities

• Receive referrals, assess hospice needs, and assign appropriate staff.
• Oversee care coordination and supervise clinical personnel.
• Review patient records, prognosis, medications, and care plans.
• Manage medical records and EMR workflow.
• Help clinicians set priorities and goals.
• Attend case conferences with hospice staff.
• Conduct quarterly record reviews and report findings.
• Assist in screening and interviewing new staff.
• Support development and evaluation of policies and training.
• Ensure compliance with professional standards.
• Assess community health needs and promote services.
• Participate in quality improvement teams.
• Perform other duties as delegated.
