Responsibilities

• Coordinate patient care and supervise clinical staff in hospice/home care.
• Review cases via home visits, conferences, and record reviews; guide clinicians.
• Maintain EMR records and workflows.
• Assist in setting goals and care plans with the team.
• Attend case conferences to coordinate interdisciplinary care.
• Conduct quarterly chart reviews and report findings to leadership.
• Support recruitment, orientation, and ongoing education activities.
