Responsibilities

• Review cases via home visits and conferences with Medical Director as needed.
• Manage medical records and EMR workflow.
• Assist in setting care goals and developing plans of care.
• Participate in case conferences to promote coordinated care.
• Participate in staff screening, orientation and continuing education.
• Contribute to policy development and quality improvement activities.
• Ensure compliance with professional standards and hospice philosophy.
